The Rise of AI Therapy: Will We Soon Say Goodbye to Human Therapists?




As we become increasingly familiar with Artificial Intelligence (AI) infiltrating our social feeds, summarizing extensive documents into concise sentences, and assisting us in learning new programming languages, a pertinent question arises: could AI replace our therapists and revolutionize mental health treatment?

The idea of an AI therapist presents numerous potential benefits. Let’s explore some of these advantages.


The Benefits of an AI Therapist


Around-the-clock Mental Health Support

In the wake of the pandemic, we have become used to the convenience of virtual appointments, effectively overcoming barriers that previously posed challenges to seeking mental health support. However, access to a therapist 24/7 could provide critical aid for individuals during their most vulnerable moments - when emotions run high and the risk of self-destructive behavior is increased. Immediate intervention and counseling could serve as an effective real-time coping mechanism to prevent self-harm or addiction-related behavior.


Anonymity and Reduction of Stigma

Engaging in conversation with strangers can be challenging for many. It becomes even more daunting when the discussion revolves around personal struggles, such as depression and anxiety, or even mental illnesses like bipolar disorder or schizophrenia. This difficulty persists regardless of whether these strangers possess professional credentials as therapists.

Also, the notion of admitting one’s inability to manage personal challenges can be overwhelming, and often results in individuals delaying seeking support for years.

Thus, the anonymity offered by AI has the potential to alleviate these concerns and encourage more people to seek the help they need. This could also enhance the effectiveness of treatment, as addressing signs or symptoms earlier often leads to better outcomes and prevents conditions from deteriorating further.


Scalability and Affordability

Nearly one billion people, or one in every eight individuals, suffer from a mental health disorder. Providing professional help to everyone is currently unfeasible and would mean increasing the number of therapists hundredfold. Moreover, training such a large number of individuals in a short time frame is impractical, and covering the cost of basic mental health treatment for every individual would be unmanageable. Consequently, we are left with little choice but to make mental health treatment more effective and automated. Does this imply that therapists will need to be replaced by AI in the future? Before we can answer this question and assess its feasibility, it’s crucial to better understand the nature of therapists’ roles, distinguishing between tasks that can be automated and those that require a human touch.


How might AI Replace a Therapist?


Assisting in Diagnosis

Consider the process of diagnosis. Intuitively, we might assume that understanding the human mind and making diagnostic assessments should be left exclusively to professionally trained therapists. However, interestingly, this is an area where Large Language Models (LLMs) could be incredibly helpful. During talk therapy, therapists often make diagnoses based on patient observations and interactions or through structured questionnaires. These interactions involve filtering a wealth of unstructured information, such as verbal communication, tone of voice, and non-verbal cues like gestures and facial expressions. Some AI models excel in processing and understanding such unstructured data. However, it’s important to emphasize that, at least in the short term, a human therapist verifying the evaluation and communicating it to the patient remains indispensable. It appears likely that the human element is crucial for effectively conveying such information.


Implementing Change

While there are many types of psychotherapy, Cognitive Behavioral Therapy (CBT) focuses on identifying thought patterns and collaborating with a therapist to change them. Although the process is more complex, for the sake of this article, we’ll maintain a simplified perspective.

Interestingly, our experiments have shown that AI is quite proficient and surprisingly accurate at identifying unhealthy thought patterns, also known as cognitive distortions. These identified patterns can then be paired with exercises, such as cognitive restructuring or exposure therapy, designed to assist the patient in altering them. However, AI might struggle to hold the patient accountable to practicing these exercises or engaging in joint practice sessions. The social dynamics inherent in human interactions play a vital role in ensuring accountability and adherence to the recommended treatment plan.

Another limitation lies in the absence of long-term memory in AI models. Although technologies to incorporate this capability exist, it still remains a challenge for systems to perfectly emulate a therapeutic experience. These systems might not always attribute the appropriate significance to past contexts or might miss some context altogether. This issue also occurs with human therapists, raising the question: Could therapy conducted by AI be equally effective, but in a different way?


Continuous Mental Health Tracking

In today’s world, we have become accustomed to tracking every calorie burned, each step taken, and every heartbeat. Yet, no continuous tracking mechanism exists for comprehensive mental health monitoring. It is somewhat perplexing that the most evidence-based tracking method still requires bi-weekly questionnaires. No smartwatch has yet been developed or a health app created to monitor our mental health in the same way we track physical fitness.

LLMs could change this. Transcripts of therapy sessions or daily journals can be analyzed to assess specific cognitive habits and transform them into trackable elements. This could enable patients to better understand their progress and allow therapists to ensure that their treatment is making a significant impact.


Conclusion

While the benefits of AI therapy are plentiful, they are not without limitations. In addition to the challenges described in this article, we must consider general AI constraints such as hallucinations, response consistency, and dependencies on APIs, which could change at any moment. We must also ponder potential sociological impacts that may be difficult to predict, such as psychological dependency on an AI tool for coping with life’s challenges.


Enhancing Treatment Efficacy through Hybrid Care

I anticipate that some of the use cases outlined will materialize in the coming months and years, while others will remain challenges for an extended period. Until these issues are entirely resolved, the involvement of a human in mental health treatment will remain vital, even for the patients themselves. There’s an inherent comfort in knowing that one is speaking to a real human - an experience that may become increasingly rare in the not-so-distant future.
